ASSISTANCE TO INDIVIDUAL IN CRISIS SITUATION (AICS)

About the Service:

  • This is a daily service intervention of Emergency Assistance Program to be extended to a walk-in client in crisis situation who comes to this office to request Financial and Food Assistance for their immediate needs such as food, medicine, transportation and special cases such as victim of abuse or disaster.

Requirements:

  1. Food- Certification from the Punong Barangay that the household head/breadwinner is sick/ deceased
  2. Medicine- Latest prescription of medicine signed by the head of the Government Doctor/Municipal Health Officer
  3. Transportation- Certificate of indigency or Endorsement from the Punong Barangay

(for special cases such as victim of abuse or disaster additional requirements is needed)

HOW TO AVAIL THE SERVICE

REFERRALS OF CLIENT TO OTHER AGENCY

About the Service:

  • This is a daily service intervention of Emergency Assistance Program to be extended to a walk-in client in crisis situation who comes to this office to request Financial and Food Assistance for their immediate needs such as food, medicine, medical procedures, transportation, burial assistance and special cases such as for victim of abuse which amount needed is higher thus in need of referral to other agency.

Requirements for

  1. MEDICAL ASSISTANCE– a. Clinical Abstract with signature and licensed number of attending physician, b. Hospital bill (for payment of the hospital)/ latest prescription of medicine/ laboratory request, c. Barangay Certificate of Residency and low income

 

  1. BURIAL ASSISTANCE – a. Promissory agreement with the Funeral Contract, b. Registered Death Certificate, c. Barangay Certificate of Residency and low income, Permit to transfer cadaver (if applicable)

 

  1. EDUCATIONAL ASSISTANCE– a. Enrolment Assessment Form or Certificate of Enrolment of Registration, b. School I.D. of the Student/Beneficiary, c. Barangay Certificate of Residency and low income
  1. TRANSPORTATION ASSISTANCE– a. Barangay Certificate of Residency and low income, b. Police Blotter (if victim of abuse, or illegal recruitment etc.

Note: If the client is in need of a home visit it will take one week to finish the case summary.

 

ISSUANCE OF CERTIFICATE OF INDIGENCY

About the Service:

  • This is a daily service intervention to be extended to a walk-in client in crisis situation or has low income earner and could not support the needed payment such as, exemption of payment.

Requirements:

  1. Certification from the Punong Barangay that income of the family is below the provincial threshold of Php 1,826.10/ member of the family or no income
  2. Certification from the Municipal Assessor that client has no landholding and property
  3. Certification from the Municipal Treasurer that client has no Business

ISSUANCE OF SENIOR CITIZEN AND PERSON’S WITH DISABILITY I.D.

About the Service:

  • This is a daily intervention of Special Social Services for Special Group and Disadvantage group
  • This service is free of charge

Requirements:

  1. For Person’s with Disability
  • Completely fill-up application form certified by the MHO
  • 2 pieces 1×1 I.D. picture

ISSUANCE OF Solo Parent I.D.

About the Service:

  • The Solo Parent I.D. is pursuant to R.A. 8972
  • Who can avail the Solo parent I.D.
  • The mother or the father who keeps the child
  • If any of the spouse is physically and mentally incapacity which is certified by a medical practitioner
  • If the mother or father is legally separated and the child is under her/his custody
  • Abandonment of the spouse at least 1 year provided that the child is under her/his custody
  • Any relative of the child who is declared by the court as the legal guardian

Requirements:

  1. Certificate of residency at least 6 months residence of the barangay
  2. Supporting Documents for Solo Parent such as Live birth Certificate of the child/ annulment document of the couple/ certification from the barangay that she is separated from the spouse/partner for at least 1 year.
  3. Complete filled up application form for Solo Parent
